What are Diabetic Foot Ulcers?

Diabetic Foot ulcers are chronic, complex, or problem wounds of the legs and feet in people with diabetes.
Diabetic Foot Ulcers that do not heal within three months are usually considered Chronic and Non Healing. Research shows that approximately 15 percent of all diabetes patients will develop foot ulcers at some point in their lives.
People with diabetes are particularly susceptible to diabetic foot ulcers because diabetes can damage nerves in your legs and feet causing problems with your blood flow, which in turn delays the process of wound healing.
What are the causes of Diabetic Foot Ulcers and Non Healing Diabetic Wounds?
Diabetic foot ulcers are caused by repetitive trauma or pressure on the foot that damage your skin. These problems can go unnoticed by people with Neuropathy (numbness in your feet) or Peripheral Vascular Disease (poor blood circulation in your legs).
Diabetes, immobilization, chronic edema, and circulatory problems contribute to the majority of chronic, non-healing wounds. Other chronic wounds are the result of traumatic injury, non-healing surgical wounds etc.
A lack of sufficient oxygen (hypoxia) in the wound bed slows or stops the normal healing process. Wound healing in people with diabetes is often complicated by poor blood circulation in the feet and legs.  Diabetic Foot Ulcers are prone to serious bacterial infections that threaten life and limb.
How do I know if  I have a Chronic, Non-Healing Wound?

If a wound has not improved significantly in four weeks or if it has not completed the healing process in eight weeks, it should be considered a Chronic Wound and a Non Healing wound.

In some extreme cases, dead tissue builds up inside and around the ulcer, which requires surgery to remove – a procedure called debridement. And if blood flow becomes too restrictive, you may require bypass surgery to help re-open your arteries. Rarely, diabetic ulcers become too infected or severe to treat.

As a last resort, amputation may be needed to stop the infection from spreading throughout the body. Half of amputations performed each year are related to diabetic wounds.

Wound care specialists classify diabetic foot ulcers using the Wagner Grade Scale:

Grade 0: No open lesions, but may possess pre ulcerative lesions, healed ulcers, presence of bone deformity
Grade 1: Superficial ulcer, not involving subcutaneous tissue
Grade 2: Deep ulcer with penetration through the subcutaneous tissue, potentially exposing bone, tendon, ligament, or joint capsule
Grade 3: Deep ulcer with osteitis (bone inflammation), abscess (pus), or osteomyelitis (bone infection)
Grade 4: Gangrene of digit (toe)
Grade 5: Gangrene of foot requiring amputation

USE & ABUSE OF REFURBISHED HYPERBARIC OXYGEN THERAPY CHAMBERS IN INDIA

Interest in new HBO facilities in India is presently expanding rapidly. This is greatly going to benefit the patients who had no other alternative treatments available to them in certain medical problems.

However, there are substantial numbers of new companies entering into the refurbished market, often in areas where they have very little knowledge of the in’s and out’s  of Hyperbaric Oxygen Therapy, neither in the technical nor in the clinical part.

The fundamental technology in a hyperbaric chamber system is fairly simple and well understood. However, the proper packaging of that technology into a safe and effective Hyperbaric Oxygen Therapy Chamber is a lot more difficult than it looks.

Buyers beware before taking a decision in buying a refurbished Hyperbaric Chamber: Consider the below bare facts !!!

1. A clinical chamber system is NOT a diving recompression chamber. The main personnel hazard in a clinical chamber, for example, is often not the much discussed issues of oxygen toxicity or fire, but the chances of components failing during the therapy creating trauma or even causing damage to the patient’s body organs.

2. Hyperbaric chambers are pressure vessels. They are subject to the boiler and pressure vessel laws of that jurisdiction which is not currently followed or known in India. This regulatory aspect has nothing to do with medicine but is hospital safety issues related to pressure vessels.

3. Hyperbaric chambers being pressure vessels, are required to be certified by (1) PVHO (Pressure Vessel for Human Occupancy) (2) ASME (American Standard for Mechanical Engineers) (3) NFPA (National Fire Protection Act.

4.With the current trend of Hyperbaric Medicine Treatment in India, Centers are opting towards the “so called refurbished” hyperbaric chambers (used equipment… strictly speaking) which are over 10-18 years old mainly from the Western and European countries.
Why these units are not refurbished? It would be prudent for any user in the USA, for example, to buy a new Hyperbaric Chamber than spending huge amounts on a new Acrylic tube, parts and the related refurbishing labour costs.

This is the reason why these “used, out dated and not refurbished hyperbaric Chambers”  are bought at junk prices and imported into India.

The underlying danger of the equipment is not explained to the Potential Customer. There are serious protocol issues related to service, availability of spare parts and most of all of the damages that may be caused  to the patient during therapy.  

5. Apart from clinical hazard, these out dated equipment may hold potential risk both to the operator and to the patient considering the aspect of dealing with highly inflammable oxygen under pressurized conditions.

Diabetic foot ulcers and non-healing diabetic wounds India is home to the second highest number of people living 
with diabetes in the world, after China. Today there are over 45 million people grappling with the disease, 
and the numbers show no signs of reducing.

Amongst other health hazards, diabetes often leads to peripheral vascular disease, involving both the macro and micro circulation. Statistics reveal that 15% of all those living with the disease will develop foot problems related 
to diabetes, and may suffer from various deformities of the toes and feet, commonly called
 'diabetic foot conditions'

These can develop from a combination of causes including neuropathy (nerve damage) and/or poor blood supply
 to the foot, which creates  areas of raised pressures in the soles of the feet, where ulcers develop. These ulcers 
act as portals for the entry for bacteria leading to serious infections, which in the already immune 
compromised patients can result in amputations or even death.

Diabetic Neuropathy can cause insensitivity or a loss of ability to feel pain, heat, and cold. This makes matters worse for those suffering from neuropathy as they can develop minor cuts, scrapes, blisters, or pressure sores that they 
may not even be aware of. If left untreated, these minor injuries can lead to serious limb and life threatening 
infections. Extensive epidemiological surveys have  indicated that between 40% and 70% of all 
lower extremity amputations are related to diabetes. This means that every 30 seconds
a lower limb is lost to diabetes. In India, an estimated 50,000 amputations occur
per year due to diabetes related foot problems. A vast majority (85%) 
of these diabetes-related amputations are preceded by foot ulcers.

Healing wounds Under Pressure

Oxygen used under pressure or Hyperbaric Oxygen (HBO2)
can assist wound healing. 

HBO2 is considered to be used successfully in hypoxic or ischaemic wounds 
such as diabetic wounds, venous stasis ulcers, failing grafts and flaps, 
necrotising soft tissue  infections and refractory osteomyelitis.

In wound healing, hypoxia is an insufficient supply of oxygen which prevents 
normal healing processes. HBO2 provides the oxygen needed 
to stimulate and support wound healing.

HBO2 is able to combat clinical infection such as gas gangrene by acting directly 
on anaerobic bacteria, enhancing leukocyte and macrophage activity and 
potentiating the effects of antibiotics.
